PP1111 - Auditory System in Children Before and After Surgery of Palatine and/or Pharyngeal Tonsils

Abstract:

Objectives: Analyze auditory system in children before and after surgical excision of palatine and/or pharyngeal tonsils.

Methods: Participants from 5 to 12 years old, divided into 3 groups: 59 children indicated to surgical excision of palatine and/or pharyngeal tonsils (G1), which 15 of those were evaluated after surgery also (G1-2) and 33 children that were not submitted for surgery and normal development (GC). The procedures were: otorhinolaryngological and peripheral and central auditory system evaluation.

Conclusion: The individuals of G1 performed the worst results in the evaluations when compared to G1-2 e GC. Children who were reassessed after surgery exhibited improved auditory responses.

Summary:

Introduction: The hypertrophy of palatines and pharyngeal tonsil can cause malfunction of auditory tube, due to its anatomical disposition and predisposition infectious affections, that could be determinant to medium otitis development.

Objectives: To analyze the central and peripheral auditory system in children before and after surgical excision of palatine and/or pharyngeal tonsils.

Methods: Participants from 5 to 12 years old, divided into 3 groups: 59 children indicated to surgical excision of palatine and/or pharyngeal tonsils (G1), which 15 of those were evaluated after surgery also (G1-2) and 33 children that were not submitted for surgery and without complaints of hearing, oral breath, and scholar performance (GC). Afterwards, the following assessments were performed: an otorhinolaryngological evaluation, speech-language anamnesis, Scale of Auditory Behaviors (SAB) questionnaire, otoscopy, pure-tone threshold audiometry, logoaudiometry, wideband tympanometry (ambient and peak pressure) and transienty and distortion-product otoacoustic emissions (ambient and peak pressure), and auditory processing assessment through behavioral tests (synthetic or pediatric sentence identification, dichotic digits, random gap detection test-RDGT).

Results: There was a statistically significant difference between o G1 and GC to measures of acoustic absorbance obtained in the of wideband tympanometry in ambient pressure in the frequencies from 226Hz to 1587Hz, and peak pressure in the frequencies from 226Hz to 1000Hz, which G1 performed lowest values of absorbance; in SAB questionnaire in the G1 performed lowest total score than GC, and in dichotic digits test in the auditory processing assessment which G1 inferior performance from GC. Also, there was statistical difference to individuals of G1-2 in pre and post phase in the auditory thresholds of audiometry for frequencies from 1000Hz to 3000Hz, in the measure of acoustic absorbance in wideband tympanometry in the frequencies from 500Hz to 749Hz, and in the performance of tests dichotic digits and RGDT in the evaluation of central auditory processing, with better results in the post-surgical phase.

Conclusion: Subjects with tonsil hypertrophy displayed altered measurements in hearing assessments when compared to control and post-surgical groups. Children who were reassessed after surgery exhibited improved auditory responses.
